Emotional wellbeing support
Compassionate listening, reassurance and practical assistance in line with an agreed care plan.
Mental health care at home can provide practical, person-centred support for adults living with mental health difficulties who may need assistance with everyday routines and wellbeing. Revive Rise offers nurse-led assessments, appropriately trained carers and coordinated support that respects dignity, personal choice and established clinical treatment plans. Care arrangements depend on individual needs, risk assessment and staff availability.

Mental health home care may help adults maintain routines, confidence, connections and independence while living with an established mental health condition or associated daily support needs.
Our assessment considers the person's preferences, communication, medication support needs, emotional wellbeing, home circumstances, safeguarding and any existing treatment or crisis plans. We can work alongside NHS and other mental health professionals where appropriate. We do not replace psychiatric diagnosis, counselling, psychotherapy or emergency mental health treatment.
Mental health home care is person-centred. Available support depends on individual wellbeing goals, assessed needs, safeguarding arrangements and staff training.
Compassionate listening, reassurance and practical assistance in line with an agreed care plan.
Support with personal hygiene, meals, sleep routines, appointments and everyday activities.
Support for prescribed medication according to assessment, care instructions and staff competency.
Help to establish practical routines, confidence and everyday living skills toward personal goals.
Assistance with meaningful activities, community appointments and social participation where suitable.
Observing agreed warning signs and escalating concerns in line with a documented support or crisis plan.
Respectful coordination with relatives, advocates and professionals where appropriate and consented.
Person-centred assistance where mental health needs coexist with disabilities or physical care needs.
Care records, person-centred reviews, safeguarding awareness and adjustments to assessed support needs.
Adults experiencing conditions such as depression, anxiety disorders, bipolar disorder or other mental health difficulties may need varying levels of help with daily routines and wellbeing. Care suitability depends on individual assessment, support goals and safety considerations rather than diagnosis alone.
We work with the person and, where appropriate and consented, family members and treating professionals to support established recovery and care plans while preserving autonomy.
Our structured approach helps families understand what will be considered before care can begin.
We learn about existing care, health conditions, daily routines and the person's preferences.
We review care requirements, risks, the home environment and relevant clinical advice.
Where suitable, a proposed package sets out tasks, responsibilities, competencies and escalation plans.
Subject to staffing and suitability, an agreed plan is delivered with ongoing oversight and adjustments.
We consider mental health home care enquiries based on individual needs, existing clinical advice, the home setting, safety planning and appropriately trained staff availability. An initial enquiry does not guarantee an immediate care package.
Care is based on appropriate professional advice and individual assessment. Specialist diagnosis and treatment decisions remain with the responsible healthcare professionals. Learn more at the NHS social care and support guide or check care provider registration with the Care Quality Commission.
